For Referrers

When referring a client to my private practice, SVM Psychology, please address the referral letter to Sathya Maniam. We kindly ask that you encourage your patients to also submit an online enquiry via our contact form or email. (Click hyperlinks). This allows our administration team to contact them promptly and organise scheduling.

Referral Letter Requirements

Under the MBS Better Access Initiative, please include:

​

1. Date of referral

2. Patient's full name, date of birth (DOB), and address

3. Presenting symptoms and/or diagnosis

4. Number of sessions referred for (e.g. 6 initial, 4 review)

5. List of current medications

6. Statement referencing a Mental Health Care Plan (MHCP)

7. Signature of the referrer

8. Provider number of the referrer

How to Send Referrals

Please email password-protected referrals and MHCP to admin@svmpsychology.au 

or via MedicalObjects — Provider No. 5528614L.

Progress letters will be provided after sessions 1, 6, and 10 and upon discharge.
